Summary
Key Responsibilities: To carry out clinical skills as appropriate to the clinical area. To participate in the running of the outpatient clinics. Accurately take and record patients weight, POC HBA1C, height, blood pressure, urinalysis and blood glucose/ketone as required. Assess the patients needs and select the appropriate blood glucose monitoring system as per patients abilities. Teach patients and/or carer how to use these devices and record blood glucose accurately in record book Participate in the training of patients to use various diabetes technologies Downloading of all diabetes technological devices Record all inpatients contacts in medical notes/Etronics patients records as appropriate and diabetes database. To provide basic education to patients with type 1 and type 2 diabetes. To complete audits as directed by the diabetes specialist nurses. Feedback to Diabetes Nursing teams anything beyond the individuals capabilities or scope of practice. Liaise with the multidisciplinary health care team under the supervision of the Diabetes Inpatient Specialist Nurse (DISN) or other Diabetes Specialist Nurses. Under the formal supervision of the Diabetes Specialist Nurses to assist with patient care as instructed Receive telephone calls from patients, carers and community staff in a professional manner. Respond where possible. Assess the need for qualified diabetes nurse to answer any telephone query and prioritise the need for this response. Record all telephone messages and information given accurately in patient records. To ensure patient confidentiality of information at all times. To assist in the maintenance of stock levels, including pharmaceutical products and maintain stock rotation. Provide support to patients and their relatives at times of distress. To be aware of the importance of a safe environment for patients, relatives and staff and to report to the nurse in charge and hazards this may endanger others and where appropriate, take action. Attend the Diabetes Team Meetings and contribute to service planning within the Trust. Update own clinical knowledge by attending study days, workshops and supervised practice with Diabetes Specialist Nurses. Attend Trust mandatory training. Use IT to support effective working To be aware of and adhere to all Trust Policies and procedures.
