Summary

The duties and responsibilities to be undertaken by the Digital Care Coordinator may include any or all of the items in the following list. Duties may be varied from time to time under the direction of the PCN management team, dependent on current and evolving PCN workstreams/priorities: Act as an ambassador for use of digital healthcare technology with both patients and colleagues. Use data to understand the health and wellbeing profiles of our population and to identify areas of unrecognized inequality and need. Identify and support those who currently do not use digital channels for healthcare access to make better use of available digital technology, promoting digital inclusion. Support PCN and practices with handling and analysing data and developing our digital maturity. Support practices to maintain and monitor data coding and quality on clinical systems, ensuring coding is compliant and consistent. Run system reports and extract data as required by the PCN for activity reporting, using EMIS clinical system, PCN digital dashboard and other data sources. Report on activity of staff employed in ARRS roles. Optimise use of PCN intranet, including document management. Be a point of contact for digital enquiries. Support introduction of new technology, including education of staff and patients. Manage content of PCN website and social media platforms. Support practices with auditing and improving practice websites. Support social media campaigns and other campaigns to promote public awareness of healthcare initiatives. Collate and develop promotional material to improve uptake of existing and new technology. Conduct audits to support development of new and existing PCN services.
