Summary

Work within the Safeguarding Childrens, CiC and the Community Paediatric Team to assist the Trust in fulfilling its responsibilities as provider of services to improve the health of our children in care Plan and prioritise own workload, ensuring sufficient time is allocated to all aspects of role Able to deputise in the absence of Named Nurse or Senior Nurse Plan and undertake statutory health assessments for Durham CiC who live in Darlington and the 20 mile radius and for those children and young people living in Darlington from other local authority areas including those children and young people with complex needs, within a legal framework and timescale Be able to formulate a comprehensive health care plan understanding the increased vulnerability of CiC that will reduce the risk of emotional harm and meet identified health needs Identify services to meet the complex health needs of CiC To undertake health summaries for all Durham young people approaching 18 years of age to promote a smooth transition into adulthood Be responsible for developing and implementing nurse-led care and interventions e.g. appropriate referral to other agencies such as Paediatrics, SaLT, OT and CAMHS Be able to work with various statutory and voluntary organisations to ensure that the health needs of CiC are supported particularly in relation to sexual health, emotional health and risk taking behaviours Establish clear and effective communication links with children/young people, parents/carers, social care & education staff and a range of other professionals including children and young people with complex needs such as language difficulties, learning and behavioural difficulties and where English is not their first language, use appropriate resources such as interpreters etc. To improve health outcomes for Unaccompanied Asylum Seeking Young People living in County Durham and Darlington Listen appropriately and take into account any sensitive issues and communicate these to appropriate agencies/persons ensuring that children are protected and that disclosures are acted upon appropriately To deliver training in accordance with the requirements of the service To keep up to date with current research and health trends relevant to the post and undertake approved training for personal and professional development to enhance the effectiveness of the post Work in accordance with Durham Safeguarding Children Partnership (DSCP) and NHS County Durham Child Protection policies and guidelines To maintain The NMC Code of Professional standards of practice and behaviour To identify gaps in service provision to Children in Care To provide health advice to other agencies i.e. local authority To be a panel member of the Fostering Service North East (Durham) Be able to interpret and advise on a broad range of health information Attend strategies, risk management meetings and CiC reviews and be able to communicate information accurately and sensitively Offer clinical support, advice and knowledge and shadowing experiences to both qualified and student nurses Undertake and participate in audits as required by the Senior Nurse CiC on both a single and multi-agency basis To ensure that the implementation of individual health plans are monitored Review the pattern of health care referrals and their outcome To ensure full registration of each child or young person and all care experienced with a GP and a dentist Be responsible for using IT equipment to deliver own day to day functions, including data generation and storage Accurately record all data and information in accordance with CDDFT policies and legal requirements, maintaining the strictest confidentiality
