Summary


An exciting career opportunity has arisen for a Community Staff Nurse to join theWokinghamCommunity Nursing team.


Within the community we are seeing more complex and acute patients than ever before, with increasingly specialist treatment and care being provided by highly skilled staff.Working within our Community Nursing TeamClinicsyou will support, assess, and treat patients with a variety of health needs, delivering high quality,evidence basedpractice. You will have a passion and enthusiasm to care for patientswithin a clinic setting.


You willbe requiredto work Monday-Wednesday (including Bank Holiday Monday’s22.5 hours).We are a friendly and supportive teamand are lookingforahighly motivatedRGN .Your role will be pivotal in ensuring the local community receive the highest quality of carewithin a clinic setting.


Please note you must currently be in the UKand have an NMC recognisednursing qualification for this role.


Working together withtheTeamto holistically assess the needs of patients and carers, and to work in partnership with the patient and family to plan, implement and evaluate programmes of nursing care.


Tobe able to undertakerisk assessmentsand take any immediate action, reporting back to the Team Leader/Clinical Leadfor further discussion and/or assessment.


Toundertake practice assessor and supervisor training support students andmore juniorstaffwith Midline Care/Maintenance and Catheter Care/Maintenance.


Provide care according to the available evidence of best practice and the support of others in the development of evidence basedpractice.


Tobe actively involved in promoting self-care for patients and building Health Promotion into care.
