Summary
Work as part of a Multi- Disciplinary Team ( MDT) to assess, formulate and support young people and their families when they are in a crisis situation and require access to mental health support.This may be in the community or in one of the acute hospitals. Offer targeted follow up interventions in a range of therapeutic modalities and training will be provided to help staff continue to build their careers. Manage mental health risk and work with the network around the young person to ensure safety and wellbeing Offer clinical assessment and evidence-based interventionwhilst observing current best practice, legislation and guidance Aim to be innovative and creative in the delivery of therapeutic intervention/care bundle, and offer psycho education to children, young people and families around their mental health crisis needs Contribute to a shared team caseload whilst considering and adhering to safeguarding policy and procedure and national and local legislation Offer support and supervision to junior colleagues and receive regular Clinical and Operational supervision to maintain practice. Plan and successfully manage smooth transitions wherethey are clinically indicated Support peers and colleagues and promote a team working environment Ensure suitable risk management and care plans areformulated for all clinical cases Maintain contemporaneous records in alignment with Trustpolicy
