We are currently seeking a dedicated and compassionate Care Assessor to join our team in Kendal and surrounding areas. As a Care Assessor, you will play a crucial role in ensuring the continuity of care for our clients while actively promoting our services and fostering strong relationships with clients, their families, and other healthcare professionals.

Responsibilities
As a Care Assessor, your responsibilities will include:
• Arranging and completing care reviews.
• Conducting new care assessments.
• Developing person-centered care plans.
• Providing guidance and care support to our care team.
• General office administration tasks.
• Participate in on-call management on a rota basis in Darwen and surrounding areas.
• Option to work 37.5 hrs paid pro rata
Requirements
To excel in this role:
• You should possess an NVQ level 3 in Health & Social Care.
• Excellent communication and organisational skills.
• Experience in a similar role
• Car driver with access to your own vehicle.
