The Prevention and Assessment (P&A) Hub has a vacancy for the post of a support planner. The P&A Hub provides short-term emergency accommodation for rough sleepers or those at risk of rough sleeping, many of whom face severe multiple disadvantages. It is a centralized base for specialized assessments, integrated care, and personalized housing support.
We are looking for a passionate Support Planner with experience of working with rough sleepers with multiple comorbidities: substance misuse, health inequalities, homelessness, offending and social deprivation. The key objectives of the service are to provide integrated care to help service users re-settle into stable accommodation.
We are looking for a passionate individual with experience of working with people experiencing severe and multiple disadvantages. The ideal candidate will be resilient, reliable and self-motivated with excellent communication skills underpinned by professional values of compassion, privacy, dignity and care. Flexible working and experience of working in a supported housing setting or similar setting is desirable.
The main function of the role include:
• To ensure the safety and security of service users, residents, staff members and visitors.
• Promote service users’ confidence, autonomy and independence.
• Complete individual support plans tailored to each individual’s support needs.
• Provide weekly support sessions, implement individual support plans and the actions set.
• Build the skills and confidence of service users to empower them in maximizing their independence
• Work in partnership with other agencies to maximize service delivery.
• Support service users to engage with a process of resettlement into stable accommodation.
• Identify and respond effectively to adult and child safeguarding concerns.
• Work in partnership with other agencies to maximize service delivery.
• Comply with the organization's policies and procedures including welfare, resident at risk and room checks.
• Maintain accurate records.
• Carryout domestic tasks to maintain the health and safety, and hygiene of the physical environment.
Please note this is a 24-hour service, you will be expected to undertake shifts from 8am to 4pm and 2-10pm (Mon to Sunday).
