We are looking for a registered children's nurse who is motivated, forward thinking and has a sound professional knowledge and excellent communication skills. You should have the ability to prioritize and work both independently and as part of a team.

In this post you will provide highly skilled, quality assessments which is outcome focused, to support children and their families, enabling them to live, participate and achieve in all aspects of life, e.g. at home and within their community. The workload is varied and interesting with opportunity to develop further knowledge and skills.

We are a very hard working but supportive and friendly team. The Specialist Nurse Children & Young People's Complex Care will have regular meetings with their team and manager to ultimately share information, improve assessment skills and learn from each other. We also support with regular training that improves and updates our knowledge and skills.

We currently work in a hybrid pattern of working, with patients’ assessments taking place in their home environment wherever possible.

Our office is in Knowsley, fingerpost area, which is central to the larger area that we cover.

Main duties of the job

The role consists of three aspects: triaging referrals, completing the comprehensive assessments and reviewing the patients that have been assessed as eligible for NHS Children and young people’s continuing care.

We are looking for Registered Children’s Nurse who has excellent communication, organisational and assessment skills, together with a sound knowledge of the NHS Children and young people’s continuing care national framework

You will be travelling to patients’ homes/or working from home and in the office.

You will need to be flexible to the various aspects of the role, have good negotiation skills and the ability to work in a calm and professional manner.

NHS Cheshire and Merseyside – the integrated care board (ICB) for the region – is responsible for planning NHS services for our population, which includes the care you receive at your GP practice, local pharmacy, NHS dentist, NHS opticians, or at hospital.

We serve a population of over 2.7m people across Cheshire East, Cheshire west, Halton, Knowsley, Liverpool, Sefton, St.Helens, Warrington and Wirral. You wil work for the ICB and although based in St.Helens will have the opportunity to assist/work within/alongside other areas.

Working alongside the wider integrated care partnership.

ICBs have four key aims:
• Improve outcomes in population health and healthcare
• Tackle inequalities in outcomes, experience and access
• Enhance productivity and value for money
• Help the NHS support broader social and economic development

To support our vision, local health and care services are working together, as equal partners, to support seamless, person-centred care and tackle health inequalities.

The ICB would expect to see the following outcomes for the ICB itself, its members and the community served:

Excellence for the communities

Assessment, planning, coordination and procurement of bespoke care packages for children with complex health

That the ICB meets individual on-going health care needs of these clients and holds responsibility for coordinating the process of securing health funding whether it is Continuing Care or Joint funding arrangements with the Local

Representation at Joint Commissioning Panels including those for SEN as required in order to agree care needs, care packages, case management and funding

Provision and of support and facilitation of the development of personal budgets for

Support and facilitation of arrangements for health assessments in-line with the national requirements for

Close and effective working relationships and processes between other NHS Trusts, General Practitioners, Primary Health Care Teams, Local Authority, Independent Providers and Voluntary Service.

Provision of activity and performance data together with specialist analysis and commentary to ICB performance reports as required.
