Summary


The Home Treatment Team based at St Ann’s hospital is looking for a Band 3 Mental Health Support Worker on a full time basis 37.5 hours per week, to join our fun, friendly and fabulous team, who have patient care at the heart of all that we do!

The Home Treatment Team East (HTTE) offers an alternative to hospital admission for service users aged over 18 with mental illness through intensive home treatment. We pride ourselves in working flexibly with patients and supporting their carers when in a mental health crisis.

This role requires you to provide a supportive role for carers and service users accessing the Home Treatment Service, carry out assigned tasks as identified by the registered nursing staff in relation to patient needs, to provide support, give time to individuals, and thus promote their
recovery and assist them to remain in the least restrictive environment.

This role will require the worker to undertake shift based work. The service hours cover the period 08.30 – 22.00, seven days per week.

To provide a supportive role for service users and carers accessing the Home Treatment Service, in order to promote their recovery.

Working with people in the community with severe mental illness who have multiple and complex needs.

To provide high quality care and treatment in conjunction with other professionals through the Trust’s agreed multi- disciplinary assessment and treatment/ care programmes.

To participate as an active member of the Home Treatment Team and be part of a Team who take pride in providing high quality and innovative services to people with severe mental illness.

Carrying out assigned tasks which have been identified by the registered nursing staff in relation to patients’ needs including visiting patients in their own home, in locations across the county of Dorset to support their recovery in line with an agreed care plan and prevent hospital admission.
