Summary

To implement and review individual personal recovery plans using the services initial assessment/review tool and specialized structured assessment tools, under the direction of Senior Nurses. To be the named key-worker managing the recovery plans for a named caseload for Non CPA service users To independently administer & monitor DEPOT injections under the direction of a senior nurse and as prescribed by a registered prescriber. To ensure service users physical, psychological, mental health and social needs are factored into assessment and recovery plans. To empower service users by promoting; hope; positive risk taking and the sense of agency to encourage self-management, self-care, personal development and life change. To provide therapeutic individual interventions to support personal recovery and social inclusion. To encourage service users to make and re-connect relationships with family, friends and peers as part of the process of promoting social inclusion. To help service users make sense of their experience of mental ill health and to normalize the expression of feeling. To work with and support carers, including involving them in the service users care, signposting to community support networks. To participate in Team Duty Rota. To actively collaborate with other teams/ services and partner agencies in shared work to support recovery plans.
