Summary
Deliver clinical and therapeutic care, within the clinical setting within set protocols and within the scope of the assistant practitioners remit. Organise and prioritise time effectively. Perform patient assessment (under supervision and after appropriate delegation from the registered practitioner), and deliver high standards of care in line with the plan of care. Implement and evaluate individual care/treatment plans. Ensure care is documented accurately and in a timely manner as per Trust policy. Support the safe admission, transfer and discharge of patients in accordance with Trust policies, under the supervision of the Specialist Nurse. Provide high quality patient care, appropriate to patient need. Take responsibility of care for a group of patients under the supervision of the Specialist Nurse. Undertake nutritional assessment and ensure the appropriate nutritional intake of all patients considering any specific dietary needs and refer to the dietician appropriately. Take personal responsibility for ensuring that patients are treated with courtesy at all times, the individual needs of the patient are respected and confidentiality is maintained at all times. Promote and value the rights, responsibilities and diversity of people. Work effectively as part of a multidisciplinary team recognising the contribution of all team members. Actively participate in team development and make suggestions to the Specialist Nurse on service improvements.
