Summary


This is an exciting opportunity for an experienced and enthusiastic therapist to clinically lead a multidisciplinary, adult neurological community service in the diverse borough of Tower Hamlets consisting of Physiotherapists, Occupational Therapists, Speech and Language Therapists, Clinical Psychologists and Support staff. The team also has very close working links with the Tower Hamlets Neuro Navigator.

Tower Hamlets Community Neuro Team (THCNT) works within the integrated framework of ‘Tower Hamlets Together’ and is supportive of innovation. There is opportunity to collaborate and lead in pathway development and quality improvement programs. There is close integrated working with a number of multi-agency stakeholders, including but not limited to, the wider Neurological service within Barts Health and also other community health services, primary care and the local authority. There is enhanced scope for collaborative working with the Neurologists at the Royal London Hospital, strengthening a neurological pathway with access to Neurological clinics. Being based in London, there is also the opportunity to enhance networks spanning a number of specialist neurological centres across London.

To provide operational and clinical leadership to the THCNT.

To support the team in identifying; managing & delivering a range of health care interventions in the community to support people in their own homes and/or other community settings.

To provide clinical / professional supervision to the Allied Health Professionals within professional field.

To provide line management to Allied Health Professionals and administrative staff within THCNT.

The post holder will be able to work autonomously and support the THCNT in undertaking assessments and planning care to support diagnosis, provide interventions, refer appropriately and lead the co-ordination of care, working with other Community Health Services, Primary Care, Acute services, partner agencies and key stakeholders.
