Summary

Are you ready to make a meaningful impact in the lives of those in need? We're on the lookout for a passionate Night Support Mentor to support our service in Kensington and Chelsea. This project as been part funded by Government End Rough Sleeping for good strategy and will provide support and accommodation to 10 individuals experiencing rough sleeping. This is an opportunity to deliver intensive, trauma informed support and interventions to enable people to stay safe and make and sustain positive changes. About the Role: We are looking for motivated and passionate individuals who are able to work flexibly to meet the needs of this complex client group. Successful candidates will be working in partnership with the local authority and street outreach teams. As a Night Support Mentor, you will support a variety of frontline care and support activities, delivering high-quality, trauma-informed services to residents and participants. Your focus will be on providing individualised support to aid in recovery, reablement, and rehabilitation within a psychologically informed environment (PIE). You will guide residents and participants through appropriate pathways and interventions, helping them achieve their desired outcomes. You will work according to service specifications and agreements, providing a flexible, responsive, and inclusive service tailored to individual needs, underpinned by SIGs Theory of Change. What We Are Looking For: Ability to use, learn and adapt to IT at an intermediate level, including Microsoft and other software programs Experience of working with people of complex backgrounds and/or a good understanding of the sector Understanding of the housing and social needs of people with multiple and complex needs Understanding and/or practical knowledge of the social and societal marginalisation that can be attached to people with complex needs Nice to Have: Experience creating co-produced support plans and providing appropriate interventions for residents/participants and liaising with other professionals Experience in providing housing support and practical assistance within a residential or outreach support role Appropriate professional qualification: NVQ/Diploma Level 2 in Health & Social Care/Psychology/Community Justice or professional equivalent Key Responsibilities: Support residents/participants in achieving their goals by contributing to support plans, risk assessments, reviews, and case meetings. Ensure effective service delivery by conducting welfare checks, risk assessments, support sessions, safeguarding checks, and necessary training and development. Empower residents/participants to make decisions to take control over their lives, by creating an enabling environment Recognise signs of deteriorating mental health, physical health, or other health implications and initiate appropriate interventions to prevent crisis or other risks Further details of the role, responsibilities, and key criteria can be found in the Job Description and Person Specification attached.
