Description
The Education Health Care Plan Coordinator (PO2) oversees and manages the implementation and coordination of individualized education health care plans (EHCP) for students with health-related needs in educational settings. This vital role ensures that students with specific health requirements receive appropriate support and access to educational opportunities. The Education Health Care Plan Coordinator collaborates with educators, healthcare professionals, and parents to develop comprehensive EHCPs that address the unique needs of each student.

Responsibilities
Evaluate and assess the health care needs of students and develop customized education health care plans (EHCPs) to support their well-being and academic success.
Collaborate with educators, healthcare professionals, and parents to create and implement individualized EHCPs that align with students' educational goals and health needs.
Ensure EHCPs are regularly reviewed, updated, and communicated to all relevant stakeholders to maintain compliance with legal requirements and best practices.
Coordinate and facilitate meetings with interdisciplinary teams to discuss and modify EHCPs as necessary, ensuring continuity of care and collaboration.
Provide training and guidance for school personnel on the implementation and adherence to EHCPs, including strategies for addressing health-related issues in the classroom.
Monitor and evaluate the effectiveness of EHCPs in improving students' health outcomes, identifying areas for improvement, and implementing necessary adjustments.
Serve as a liaison between educational institutions, healthcare providers, and families, promoting effective communication and collaboration to meet the unique needs of students with health conditions.
Requirements
Bachelor's degree in a relevant field such as Education, Nursing, or Health Sciences.
Proven experience in developing and coordinating health care plans for students in educational settings.
In-depth knowledge of applicable laws, regulations, and policies related to education health care plans.
Strong interpersonal and communication skills, with the ability to collaborate effectively with stakeholders from diverse backgrounds.
Excellent organizational skills and attention to detail to ensure accurate and comprehensive EHCP documentation.
Ability to provide professional development and guidance to school personnel on implementing EHCPs.
Familiarity with healthcare terminology, conditions, and procedures to accurately assess students' health needs and develop appropriate plans.
