Summary

Along with the Manager, be responsible for the day to-day operational functioning of the Team. To deputise for the Manager in their absence. Participate as an active member of the Team, accountable for your practise and work in partnership with other health care professionals, patients and others in the delivery of care. This involves assessment of care needs and the development, implementation and evaluation of care plans / care programs and the setting of standards of care. Plan, prioritise and manage the delivery of a health and social care assessment and home treatment service for those in crisis as an alternative to inpatient admission. This includes managing and prioritising referrals and ensuring staff attending have suitable skills and experience and work is matched to level of experience and skills. Support team members in the management of particularly complex or distressing cases jointly visiting with them and reviewing care plans. Demonstrate within the Team, clinical leadership and knowledge of a range of therapeutic interventions available to reduce distress for individuals experiencing an acute mental health crisis. These can include: Supportive Counselling Social Systems Interventions / Family Work Brief Interventions / Problem Solving / Using CBT techniques Medication (with education and support) Practical help and support at home for patient / carers Health Promotion Co-ordination of the patients care and ensuring they / carers have adequate support from other services and are receiving benefit entitlements Act as a role model in the practise area, encouraging staff in their practise of thorough assessments with information from others, involving the patients social system in care planning and thinking creatively with others to find solutions. Ensure continuous discussion and overview of Team members work to encourage reflection / team review to keep standards high and help staff feel well supported. Ensure the provision of Clinical Supervision to all grades of staff (via structures for individual supervision and availability of group supervision) and be responsible directly for supervision of Band 6 workers. Facilitate a learning environment within the Team by arranging educational slots for Team members and others to contribute to. Ensure staff members have access to study time and appropriate training courses and maintain training records via supervision and appraisal. Be responsible for the maintenance of accurate recording of patient information in the documentation and IT systems by ensuring there is clear guidance for staff on documentation that provides accurate, relevant information without too much repetitive paperwork. To closely monitor and review this regularly. Ensure compliance with the Care Program Approach, the Trust policy on assessment and management of risk and the Mental Health Act by ensuring staff have adequate knowledge of all of the above (ensuring training if necessary) and reviewing their practise. Provide specialist knowledge and advice in the areas of Crisis Resolution / Home Treatment, Risk Assessment & Management and Social Systems Approach to other staff both within and external to the service. Contribute to a safe working environment by ensuring staff are aware of policies for home visiting which are regularly reviewed. Supports and facilitates multidisciplinary audit, development of care pathways and protocols and other quality improvement processes. Promote team cohesion via facilitating positive group dynamics. Comply with the legal requirements concerning the prescribing, storage, security transporting and administration of medicines COMMUNICATION: Possesses excellent communication skills and is able to form a positive therapeutic relationship with people who are in crisis/ experiencing acute distress in the context of partnership and trust. To be aware of how your verbal and non-verbal communication can be interpreted by others and maintain a polite approach at all times. To be able to respond positively to advice and constructive criticism. To be able to listen well, ask open-ended questions, reflect back to patients and their carers and summarise their needs. To promote good communication within the Team, encouraging members to express their views and be listened to, fostering an environment of discussion and reflection on patients care always looking to maintain and heighten standards. To appreciate the role of humour in promoting team cohesion. Demonstrates good leadership and management skills when undertaking the clinical assessment process and delivering care. Promote good communication / multidisciplinary working with other Teams / colleagues by ensuring their role is appreciated, working jointly, keeping the focus on the client and resolving any difficulties that may arise. Liaise closely and effectively with colleagues in primary and secondary healthcare services, families and carers, social services and voluntary agencies etc to ensure well co-ordinated and consistent care for people being treated by the HTT. Ensure patients and their carers / significant others are always fully involved and informed in the process of assessment and care planning. Ensure patients and their carers have a full understanding of how the team works and how to contact them (both from verbal and written information) and there is a system in place for them to give full feedback. To ensure each member of the Team has access to effective individual / group clinical supervision and to directly provide supervision to senior staff (as well as being supervised by the Manager). Ensure the Team Manager is kept fully briefed of clinical issues and concerns. To ensure that information of a confidential nature is not divulged to any unauthorised person and is stored in line with the provisions of the Data Protection Act. Ensure that all documentation is legible and written is accordance with professional guidelines and best practise. Participate in the planning and facilitation of the induction and processing of educational programmes for all staff. Participates in and develops audit and research in relation to Crisis Resolution working. Act as a mentor for students on placement and provide education and support. Be a source of advice / education for other teams regarding Crisis Resolution / Home Treatment working PERSONAL DEVELOPMENT / TRAINING: To keep up to date and participate in training programmes identified in a Personal Development Plan. To ensure self knowledge of changes in health care Participate in formal appraisal system in accordance with Trust Policy and participate in clinical supervision. Participate in (and promote) special projects, research and surveys in relation to HTT working. Ensure all staff are aware of their mandatory training requirements so that professional and legal requirements are fulfilled. Ensure that an active in-house education program exists which ensures that all staff are able to carry out their role to a high standard and allows them to develop skills and competencies in interventions whose efficacy is supported by research evidence. Provide education and training within the Team (and to other Teams in the Trust and external where time allows) in relation to his / her sphere of specialist knowledge. Ensure that professional and contractual standards of care are known to all staff and that there is a system of monitoring those standards and a system of audit. Maintain up-to-date knowledge of evidence regarding nursing and multidisciplinary practise and Crisis Resolution Team working. Ensure this is shared with and by the Nursing Team. To participate in the recruitment and interviewing of new staff and ensure they have an orientation program and supervisor on starting.
