We are currently seeking a dedicated Mental Health Practitioner to join our Chorley Community Mental Health Team. This role presents an exciting opportunity to positively impact the lives of individuals facing complex and enduring mental health challenges. Through innovative approaches like Dialog+, we collaborate closely with service users, understanding their experiences, identifying goals, and providing tailored support to empower them on their journey to wellness.

As a valued member of our team, you will have the autonomy to deliver high-quality care while also benefiting from collaborative efforts. Your responsibilities will include connecting service users with vital resources, offering psychoeducation, and co-creating practical coping strategies to help manage symptoms effectively.

Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.

The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.
