Summary
Key responsibilities Develop and maintain a strong, positive relationship with children and young people with a learning disability, autism or both and to ensure that they and their families get the right support at the right time Provide support to access the right help at the right time across complex systems particularly the transition between children and adults services Build relationships and support coordination across the system, including Health, Education and Social Care Hold services across the system to account where action plans are in place or actions have been agreed Ensure transitions between placements, settings and services are well managed Keyworkers will be trained to apply the CNEST Risk Rating Tool and will work with other professionals across the system to identify children and young people who meet the criteria and would benefit from oversight via the Tracker meetings To facilitate, participate in and follow up actions from Care Education and Treatment Review (CETR) meetings. Champion Autism, raise awareness and promote strategies, increasing understanding. Troubleshoot and challenge on behalf of the child/young person and their family. Produce case studies, evidence outcomes and contribute to collection of data and feedback.
