You will need to register if you do not already have an account. 1.1 Participate in the process of holistic care delivery to service users.1.2.Assist and support service users in developing a healthy and balanced lifestyle.1.3.Promote and assist/ support service users taking advantage of opportunities for leisure and productivity.1.4.Support service users with activities of daily living and in accessing community resources.1.5.Support registered health care staff and service users in implementing risk management and care plans by assessing the service users response to activities.1.6.To recognise service users strengths and resiliencies that offset risk and promote their use as part of a pre agreed risk management plan.1.7.Ensure care is always carried out to the highest standards and within the Policies and Procedures of the Trust and to national guideline standards.1.8.Ensure attendance at Team Meetings and training and professional development.1.9.Provide effective communication with clients, staff and other agencies within the Care Programme Approach.1.10. Maintain accurate records and ensure a secure method of record keeping and professional exchange of information while maintaining confidentiality.1.11. Report any significant change in service users verbal or non-verbal behaviours to the shift coordinator immediately.1.12. The post holder will contribute towards the delivery of a quality rehabilitationservice and well-being, working as a member of the multi-disciplinary team with the Occupational Therapy and Nursing staff.1.13. Build awareness of the recovery philosophy and the social inclusion agenda, promoting autonomy and enabling choice.1.14. Plan and carry out individual and group interventions within a specific remit (e.g., sporting, creative, domestic or technical activities). Most of the planning is straightforward but there may be a requirement to adjust plans at times.1.15. Undertake a range of activities, working flexibly in accordance with the needs of service-users and the service as a whole.1.16. Maintain dedicated time for practical groups and demonstrate effective time management when preparing activities and writing notes.1.17. Assist in facilitating service user responsibility and choices for occupational participation, and the ability to determine their own lifestyle.1.18. Contribute to occupational and clinical risk assessments, reporting findings to the occupational therapy team.1.19. Assist in the co-ordination of care by communicating effectively and ensuring that Trust professional guidelines for record-keeping are followed.1.20. Ensure that assessment and treatment is delivered in a person-centred manner ensuring the gender, sexual orientation, ethnic, cultural and spiritual needs of service users are adhered to.1.21. Participate in a multi-professional/agency approach to care, valuing the interface of professional roles and responsibilities.1.22. Use effective interpersonal skills that relate to service users, carers and team members on an equal and respectful basis.1.23. Demonstrate awareness of the Trust Policy for Clinical Supervision; receiving regular clinical supervision.
