Summary
Your team and role We have teams based at five major hospitals in Hampshire: Queen Alexandra Hospital in Portsmouth, University Hospital Southampton, Royal Hampshire County Hospital in Winchester, Basingstoke and North Hampshire Hospital and Frimley Park Hospital based on the Hampshire/Surrey border. Each team has a Social Work Discharge Team. Our teams work in a fast-paced multi-disciplinary environment, working closely with our colleagues in Health, Social Care, providers, and the voluntary sector to ensure a safe, timely and sustainable discharge plan. We focus on supporting the recovery and rehabilitation of individuals, identifying long term needs, safeguarding and maximising independence. We work across a range of sites including acute hospitals and community hospitals to plan onward care for people in their own homes as well as short term and longer term care services. Our professional and highly dedicated teams offer a supportive space for colleagues to grow and develop. Your role As an Assistant Team Manager for Adults' Health and Care, you'll provide person centred, safe and secure services through the enactment of social care legislation and local government requirements, informed by professional training. Your role will include supporting the manager with the day to day running of the team including case allocation, supporting and advising the team when managing complex cases, managing staffing levels and rotas and offering regular supervision with staff. You'll also represent the organisation in multidisciplinary meetings, support with safeguarding decisions and manage the triage and onward care functions.
