Summary
Lead on the development and implementation of a learning disabilities pathway within the prison and be the lead expert for learning disabilities across the service. Have a key role in supporting the training of key stakeholders within the prison.Responsible for the assessment, planning, implementation and evaluation of evidenced based interventions. Manage a caseload, practicing within the framework of the multi-disciplinary team and Care Programme Approach. Provide a care coordinator role and communicate effectively with other professionals, internal and external to the Inclusion Team and prison, to ensure throughcare support where appropriate.Work with people from initial assessment through to discharge which can include transferring care to other providers such as hospitals or CMHTs whilst working with a variety of agencies in the Offender Health Pathway.
