Summary

Clinical Perform comprehensive assessment of patient health needs in early detection of IBD To provide a specialist nurse led service working autonomously in providing care for IBD patients and their families according to agreed protocols or as specified by the Consultant Gastroenterologists. Plan, implement and evaluate care delivery according to changing health care needs Perform advanced clinical skills in assessment &/or diagnosis &/or treatment Involve patients and carers/relatives in the planning and delivery of care and development of services. Monitor investigation results, initiating further assessment/review of treatment pathway as required. Monitor and manage patients whilst on treatment, including Professional Development To be have a PDR annually and contribute to own personal development plan To maintain and develop leadership, clinical competence and professional awareness in accordance with CPD and revalidation requirements and to maintain a professional profile To take part in reflection and appropriate learning from practice, in order to maintain and develop competence and performance Improving Quality and Developing Practice To develop valid and reliable methods to continually evaluate and audit own practice and service outcomes, identifying, interpreting and acting on audit outcomes To maintain records of activity including clinical coding, maintain appropriate documentation and participate in audit of documentation, clinical activity and outcomes To use and develop information systems to support decision making and improve care To maintain knowledge of contemporary epidemiological, demographic, social, political and professional factors relevant to the specialist area of practice Developing Self and Others To participate in review of own practice with colleagues from own and other professions thorough case review and clinical supervision To develop and provide patient education and information which is appropriate and responsive to individual needs and level of development To participate in the development of evidence based protocols, standards, policies and clinical guidelines To evaluate knowledge and practice, identifying learning opportunities and continuing development through appraisal and personal development planning. Research and Audit To actively participate in clinical audits and monitor clinical effectiveness of the role. To use audit results to promote and drive change as appropriate.
