The postholder will provide collaborative leadership, system and sustainable transformation to deliver the ambitions of the Be Well Domain and the Alliance Delivery Plans. The Ipswich and East Suffolk Alliance (IESA) follows the Live Well model and this programme is an integral part of the Be Well Domain to empower adults to live healthy lives. The postholder will work as part of the IESA Partnerships team alongside system colleagues, the wider CONNECT space incorporating the Voluntary Community Faith and Social Enterprise (VCFSE) sector, other public bodies such as Police and Education to plan, oversee and develop programmes for the benefit of our communities. The role will oversee a diverse suite of projects aimed at delivery of integrated place-based care across seven localities. The postholder will continue to embed existing personalised care, health inequalities and partnership projects already underway as well as develop new plans. The post holder will be responsible for embedding Personalised Care into every NHS priority workstream across IESA in accordance with the SNEE ICS Personalised Care Strategy and oversee a diverse suite of projects aimed at delivery of integrated place based personalised care across localities and workstreams. The post holder will be responsible for all aspects of programme management and delivery of projects ensuring governance and monitoring and evaluation is in place for all programmes including health inequalities, personalised care and social prescribing. The postholder will continue to embed existing Personalised Care and health inequalities projects already underway as well as develop new plans to ensure sustainability and scalability. The post holder will be responsible for engaging with key stakeholders across Ipswich and East Suffolk, ensuring clinical engagement, to implement the six components of the personalised care model and various prevention models. To successfully deliver our programmes, the post holder will need to work with key partners on changing the relationship that people have with healthcare practitioners to become equal partners with the necessary level of skills, knowledge and confidence. The post holder will need to ensure the Alliance workforce strategy supports this change. The post holder will work as part of IESA and will be expected to work closely with social care, district council, voluntary and community sector and other colleagues delivering prevention and personalised care working towards an integrated approach. The Programme Manager will develop a robust framework by which to manage, monitor and report on all Partnership programmes including Personalised Care and health inequalities across Ipswich and East Suffolk. There will be a need to demonstrate impact on the person and the system identifying what good looks like, building a local evidence base. The Programme Manager will deputise for the Deputy Director of Partnerships and Alliance Delivery at Board level and sub groups.
