Summary

Detailed job description and main responsibilities Monitor in- and out-patient HIV/Sexual Health and Infectious Diseases (ID) prescribing Participate in the provision of clinical pharmacy services to HIV and ID in-patients on ward B6 (the Clinical Infection Unit), and other wards in BSUH, including participation in consultant and registrar led ward rounds Provide patient-specific advice regarding drug treatment and make treatment recommendations to prescribers in order to optimise drug treatment (for both in- and out-patients) Attend and participate in weekly multidisciplinary team meetings and the weekly viral load meeting (in which treatment options for HIV patients are discussed by an expert panel) Undertake detailed medication counselling and adherence support for patients considering, starting, taking or changing ARV therapy Practice (if qualified) as a pharmacist Independent Prescriber in dedicated pharmacist-led outpatient clinics Be responsible for clinical continuity of care within the outpatient setting Provide specialist clinical pharmacy expertise and medicines information to the HIV/SHAC MDT, to clinical staff from other medical specialities and to other pharmacy staff Provide advice to staff in primary care in particular regarding drug-drug interactions with antiretrovirals Please see full job description and person specification for more information
