Summary

Job Summary Background Information The Unpaid Carers Programme sits within the Long Term Conditions and Personalisation function which includes programmes of Diabetes, Stroke, Cardiovascular disease, Palliative End of Life Care, Unpaid Carers and Personalisation. The Unpaid Carers Programme aspires to ensure unpaid carers are recognised and given the support they need. This includes support to manage their caring role and remain in work and/or education, regardless of where they live in West Yorkshire. The role will be responsible for identifying and developing ways to support the existing and future workforce who are balance caring responsibilities, signposting to additional support available for unpaid carers and agreeing unpaid carers service standards all organisations across West Yorkshire. The unpaid carers programme aims to amplify the voice of unpaid carers and ensure they are supported to fulfil their caring responsibilities whilst also looking after their own health and wellbeing. The role will focus on leading and co-ordinating specific workstreams in order to optimise recognition and support for unpaid carers across our health and care system. The postholder will work with stakeholders across all organisations within the Partnership including local authorities, education and health, developing effective partnerships to support making demonstrable improvements to the lives of unpaid carers. The role will also require significant involvement of and engagement with voluntary and community sector (VSCE) organisations, primary care and social care to ensure optimal advocacy, support and recognition of the valuable contribution of unpaid carers across our health and care system. As a champion of organisational change and service improvement, the post holder will require high levels of self-motivation, tenacity and resilience. Success in the role will necessitate the establishment and development of strong and effective working relationships at all levels, with the ability to challenge and effectively support the delivery of complex, multifaceted workstreams, through the application of structured project management processes and interventions. The Project Manager for Unpaid Carers and Personalisation will: Deliver a robust platform to amplify the voice of unpaid carers in order to inform our priorities as a programme Provide timely and co-ordinated project management to the Unpaid Carers Programme Work collaboratively with local and national networks to produce resources to raise awareness, upskill professionals and support better identification and support for unpaid carers Embed personalised care approaches through all areas of work including empowering unpaid carers to have choice and control, to be part of shared decision making conversations and support the person they provide care for to be better equipped to manage their condition together with support of health and social care teams Working with stakeholders to identify barriers and creating opportunities to amplify the voice of Black, Asian and Minority Ethnic communities in addressing key issues Developing innovative resources to raise awareness and reduce stigma of unpaid carers across our health and care system Develop a model of engagement to encourage participation across our local communities and networks Explore and implement the successful delivery and uptake of digital solutions to support the health and wellbeing of unpaid carers across West Yorkshire Establish clear links and relationships to other projects and programmes Provide a link between place based teams supporting unpaid carers to enable sharing of good practice including case studies and resources Maintain a central point for West Yorkshire good practice resources Identify and systematise good practice in supporting people to increase awareness of what good looks like for unpaid carers across the Partnership in delivery of service provision and outcomes including the use of relevant measures and assessments in West Yorkshire Provide support to spread and embed all elements of unpaid carers and personalisation strategic priorities through all places in West Yorkshire, including within the other programmes in the function of Diabetes, Stroke, Cardiovascular disease and Palliative End of Life Care.
