Summary

Main duties and responsibilities The post holder will be expected to lead, participate and develop clinical pharmacy service provision at the Haywood Hospital. They will be responsible for implementing and monitoring the Trusts clinical pharmacy standards as well as ensuring the clinical service is of high quality and patient focused. They will identify gaps and problem areas in the service and use appropriate methods including quality improvement to improve the service. Lead on service evaluation and audit of all aspects of the medicines management/Pharmacy service supporting internal and external benchmarking activities including the development of KPIs indicating service delivery outputs. Revision to clinical pharmacy service provision across hospital and community based services ensuring that clinical service provision is optimised and that service users have the opportunity to discuss their medication with a member of the Pharmacy & Medicines Optimisation Team. Clinical supervision and line management of pharmacists to ensure safe and effective service delivery which meets the needs of service users. Provide clinical pharmacy input at ward level and / or dispensaries, to ensure the safe and appropriate use of medicines in order to maximise benefit and minimise risk to patients. This includes regular review of prescriptions, identification of significant drug interactions, advice for clinical monitoring, compliance with legal aspects of medicine supply and compliance with the Trust formulary Provide clinical leadership and advice within the organisation and externally e.g. homecare companies regarding medicines optimisation of biologics and pharmacy services, working collaboratively with specialised NHS commissioning, pharmacy homecare team and clinical networks. Accountable for operational delivery of pharmacy services to all clinical services in North Staffordshire and services provided through Haywood hospital pharmacy, either through direct provision or through establishing professional input into contractual arrangements and management The robust, safe and effective operation of all pertinent Pharmacy services within the Trust, ensuring compliance with Medicines legislation, identify and support new developments, including how pharmacy can support new Trust initiatives, including long-term service planning and new ways of working Support the Medication Safety Officer in the implementation of safety alerts and lead other governance issues related to clinical pharmacy services within areas of responsibility. To feedback to the relevant speciality, governance issues like incident trends, development issues etc. and input into the governance plan Works as a core member of agreed Trusts Medicines Committees, and acts as its representative at other local health economy-wide committees. Development and delivery of training and continuing professional development of pharmacy staff (pharmacist and technical), nurses, doctors and other staff groups in clinical speciality in aspects pertaining to clinical pharmacy as required to meet identified training needs To ensure that clinical pharmacy services development supports the Medicines Optimisation strategy and is linked to the strategic direction of travel for the Staffordshire and Stoke on Trent Care Group. To provide the Deputy Director with details of output, performance including clinical interventions and activity at specified time intervals in an agreed format.
