Summary


Biological and immune-modulating medicines (biologics) have fundamentally altered the treatment landscape for a wide range of auto-immune and inflammatory diseases. We are looking for an enthusiastic and dynamic pharmacists to join our pharmacy team and work within our clinical teams to help ensure the safe and effective use of these medicines within North Derbyshire to effect real changes in people’s lives.

Working with clinical teams within rheumatology, gastroenterology, ophthalmology and dermatology services you will support the inpatient and outpatient provision of biologics. The role will enable you to develop your clinical, leadership and management skills to the highest level. You will be supported to help develop your advance practice skills as well as project management and financial reporting.

Good communication, time management and analytical skills will be required and the ability to work effectively and communicate within a team. You will also contribute to the provision of weekend pharmacy services.
Applications are welcomed from pharmacists who will complete their post graduate studies in clinical pharmacy in 2024.

Working within a multidisciplinary team you will help to ensure that patients are appropriately assessed, including review of relevant laboratory monitoring, and fulfil the relevant commissioning criteria to initiate treatments, through the blueteq system. You will help co-ordinate and validate the repeat prescribing of biologic medicines, including through homecare provision, and the counselling of patients.

You will develop your prescribing practice to begin repeat prescribing of these medicines against set criteria. You will have the opportunity to contribute to MDT discussions and complex case review, both in terms of a learning opportunity but also to utilise your pharmaceutical knowledge.
You will also help lead biosimilar switches by supporting the clinical teams and through direct patient contact.

There are also opportunities to undertake clinical supervision and mentoring, service evaluations and audits against relevant criteria and standards (e.g. NICE, RPS Professional Standards for Homecare Services) to develop your skills and aid your career progression.

Prescribing and verification of biologic prescribing

1. To lead the provision and development of clinical Pharmacy services to relevant specialties

2. Provide support and oversight of specialist medicines use within relevant specialties
- Contribute to eligibility assessment process before initiation of new therapies
- Undertake and oversee the addition of patients to BlueTeq where necessary
- Clinically validate biologic prescriptions
- Clinically validate intravenous day case prescriptions
- Facilitate supply via homecare for appropriate patients and provide ongoing support as needed
- Provide counselling for patients on new biologic medicines
- (B7) Undertake repeat prescribing for standard therapy biologic medicines
- (B8a) Undertake wide range of biologic prescribing and where appropriate IV therapies
- Coordinate repeat prescribing for biologic medicines
- Request and where appropriate interpret any blood monitoring necessary as part of the
prescribing process
- (B8a) Request and review reporting from any relevant imaging
- Review and interpret disease scores according to commissioning requirements
- Contribute to the clinical review of patients on biologics
- Support biosimilar uptake as needed
3. MDT
- (B8a) Lead medicines related discussions at specialty MDT and provide specialist advice on
medicines optimisation issues
- (B7) Contribute to speciality MDT and provide specialist advice on medicines optimisation issues

Guidance and protocols
4. Support the review of specialty requests for new medicines. Including formulary management and new product requests for Drug and Therapeutics Committee. Provision of expert advice and
information by evaluating and interpreting published evidence.
5. Facilitate the development and implementation of medicines related guidelines
6. Support quality improvement and audit relating to medicines
7. Provide training to speciality staff.
8. (B8a) To monitor compliance and advise on the implementation of National Guidance e.g. NICE
9. (B8a) Consult and lead on local pathway adaptations through liaison with ICB colleagues
10. (B8a) Provide pharmaceutical representation at appropriate specialty meetings.
11. (B8a) Coordinate the review of medicines related incidents within the speciality
12. (B8a) Monitor prescribing within the teams to identify training and information need.

Finance
13. Contribute to any requirements for financial reporting e.g. uptake of biosimilar treatments and
associated savings.
14. Provide clinical pharmacy oversight for the CRH homecare team, support with KPI and address
clinical queries
15. Work with specialist teams and within pharmacy to reduce waste of high cost medicines.
16. (B8a) Review monthly invoice reporting and highlight any anomalies
17. (B8a) Contribute clinical intelligence to the annual budget setting process
18. (B8a) Contribute to the high-cost drugs and homecare meetings
19. (B8a) Represent CRH at ICB biosimilar working group
20. (B8a) Maintain links with other acute providers to establish shared learning and best practice
21. To provide training to pre-registration students and pharmacists, including tutoring for the MSc/Diploma in clinical pharmacy.
22. To provide training and education to other healthcare professionals.
23. To provide professional supervision to technical and supporting staff and to undertake the role of Responsible Pharmacist when required
24. To contribute to the delivery and development of clinical pharmacy services across the Trust
