Summary

An exciting opportunity has arisen for a registered Occupational Therapist/Social Worker/or Registered Mental Health Nurse or equivalent professionalto play a pivotal role in delivering this service, as well as planning, evaluating, and contributing to its development. We are looking fora professionalwho is driven, passionate and excited to be part of a strategically important service provision, in line with our programme of Community Mental Health Transformation which is aiming to transform and enhance the way we deliver our mental health servicesand aligned with the Trust’s vision as ‘One-Team’.
The service will employ an innovative model to provideout of area service delivery, gatekeeping for S117 after care applications, out of area placements/independent hospitalsand individual funding requests for specialist complex mental healthcareforpeoplewithsevere mental health issues. The role will offeranopportunity to make a real difference to people’s livesbyworking very closely withthe mental health locality teams,ICBs, Local Authorities, CareProviders,and other key stakeholders.
The role will offer autonomy,variety,and significant development opportunities, both in leadership and clinical skills.
To provideasafe and effective provision of Out of Area Placements (OAPs) services on a daily basis. To ensure that Out of Area Placements deliver Safe, Effective, Caring, Responsive and Well-Led service with desired outcome as per care plan.
This will includedelivering a serviceto enableservice usersto live healthy, fulfilling livesin a least restrictive manner as well as working across boundaries in a seamless manner to maximise a ‘value for money’care provisionto achieve the best possible outcomes for our service users.
