Summary


We are excited to share an opportunity to join our team of Elderly Care consultants here at CHFT.

Our Elderly Care services have recently been developed to bring the teams together and improve the care of patients in our local community. Our services are currently evolving, and by joining us at this exciting time it will give you the unique opportunity to lead the development of new services across hospital and community settings. Our future plans and transformation projects include the further development of a Virtual Ward and Frailty SDEC, as well as the reconfiguration of our hospital sites.

In recent years the service has developed to deliver several sub-specialty services, including Falls, Parkinson’s and Orthogeriatrics, and is currently expanding it’s Virtual Ward Services within Frailty. The HRI site benefits from a Frailty Same Day Emergency Care (SDEC) unit staffed by Specialist Frailty Nurses, Frailty Advanced Clinical Practitioners and Geriatricians.

The geriatric medical team manages a substantive bed base of 103 acute elderly care beds. Consultants currently work in a model consisting of 2 consultants per ward which provides a consistent approach to patient care, as well as the ability to cross-cover periods of leave. In addition, there are 16 separate frailty beds on the acute medical unit at HRI. The team also covers Virtual Ward and Frailty SDEC.

We employ more than 6,500 staff who deliver compassionate care from our two main hospitals, Calderdale Royal Hospital and Huddersfield Royal Infirmary as well as in community sites, health centres and in patients’ homes. We also are incredibly proud to have almost 150 volunteers here at CHFT.

We provide a range of services including urgent and emergency care; medical; surgical; maternity; gynaecology; critical care; children’s and young people’s services; end of life care and outpatient and diagnostic imaging services.

We provide community health services, including sexual health services in Calderdale from Calderdale Royal and local health centres. These include Todmorden Health Centre and Broad Street Plaza.

We continue to modernise and invest in our health services to build on our strong reputation. Foundation trusts are public leaders in improving quality in health services. They are part of the NHS – yet decisions about what they do and how they do it are driven by independent boards. Boards listen to their Council of Governors and respond to the needs of their members – patients, staff and the local community.
