Our friendly District Nursing team is looking forBand 5 Nurseto support their team inBracknell.


Your role asa Community Staff Nursewill be pivotal within the Community Nursing teamsto


provide planned careandto deliver a seamless service to patients. The Band5 staff nursewill work with other team members embracing case management and theLong TermConditions Agenda thus preventing avoidable hospital admissions andfacilitatingappropriate earlierdischarges into the community.


To give skilled nursing care, based on principles of best practice and clinical evidence.


Undertake comprehensive assessment of the physical,psychologicaland social care needs of patients including the needs of carers. To provide individualised andperson centredcare plans that provide monitoring and promotion ofself careto prevent unnecessary hospital admissions


Act as a Named Nurse with the support and supervision of the DN Team Lead. To take continued responsibility for the holistic health needs assessment including care planning, evaluation andappropriate implementationof health care and ensuring continuity of care following referral from acute care, rehabilitation units,GPsand other agencies.


Act as the patient’s advocate and tofacilitatethe patient’s own choiceswith regard tonursing care, promoting independence andself care, asappropriate.


Toidentifyand recordnew problemsand other relevant information reporting back to the Band 6 asappropriate.


Maintainaccurate,comprehensiveand contemporaneous recordsin accordance withTrust and NMC guidelines.


Be alert to the needs of vulnerable adults including risk assessment and takingappropriate actionwhenrequired.


To develop and utilise specialist nursing skills such as leg ulcer assessment and management, syringe driver set up, intravenous drug administration, continence assessment, and palliative care.
