Summary


An exciting opportunity has arisen for 2 Band 5 staff nurses to join the Neonatal Continuing Care Team within our Tertiary level 3 Neonatal Unit at Hull University Teaching Hospital. The post holder will be welcomed in to a substantial and supportive team.

We are looking for enthusiastic, highly motivated specialist registered nurses who wish to further develop their knowledge and skills in the community setting. The successful applicant will play a vital role in delivering high quality safe and efficient care to preterm, low birth weight babies in the community setting following discharge from the Neonatal Unit or Transitional Care.

The post holder will work as part of the Neonatal team facilitating the safe discharge and follow up of patients in line with Neonatal guidelines and trust policies. Working in partnership with the Multi Disciplinary Team (MDT) to actively determine the best place for continuing care of each baby, whether this is staying on the Neonatal Unit, Transitional Care Unit, home or paediatric ward. Ensure that from admission any relevant medical history is collected from family members and therefore be able to ascertain what support families may need both during admission and follow up in the community.



The post holder will use their specialist knowledge and experience to support the transition and adjustment from an inpatient stay to community setting by working closely with community partners in promoting positive and equitable engagement with patients, families and carers. This includes the assessment, planning, development and implementation of care plans in conjunction with family/carers.



A part of this role you will also work closely with the neonatal team, Paediatric community services, palliative care teams, and community midwives, health visitors and the wider MDT. Promote a seamless transfer of care from hospital to home.
