Summary
1. To support patients, their carers and relatives by providing up to date information on secondary prevention (which meet British Association of Cardiovascular Rehabilitation and Prevention Standards). 2. To communicate complex patient related information effectively, to ensure collaborative working with the multidisciplinary team members across health and social care sectors ensuring the delivery of a co-ordinated multidisciplinary service. 3. To use a range of verbal and non-verbal communication tools to communicate effectively with patients to progress rehabilitation and treatment programmes, and to ensure an understanding of their condition/problem so that patients are supported in self -management of their condition. 4. To promote patients self-management of their condition by reducing risk factors through medication adherence, smoking cessation, increasing functional capacity, weight and general diabetes management, and thereby reducing disease progression and hospital readmission. 5. To utilise a variety of strategies to communicate effectively with patients and those involved in their care. The post holder must be able to communicate effectively both in written and in oral form. To keep updated with current and latest treatment, research, and clinical care. 6.
