Summary
To facilitate the most appropriate discharge pathway for patients leaving hospital To support cross organisation working and information sharing To act as a single point of access to community services for patients who have been admitted and/or are needing support on discharge To share information and signpost people to the most appropriate service To co-ordinate the day to day running of IRIS at Frimley Park Hospital To facilitate appropriate information sharing in relation to supporting patient discharges To triage referrals received in IRIS, forwarding them to the correct service for the patients discharge To use the EPIC system to track patients and pull reports
