Summary
An exciting opportunity has arisen for experienced Support Time & Recovery Workers to join Lancashire care Pennine Home Treatment Team within the Hyndburn, Rossendale, Ribble Valley, Blackburn with Darwen Burnley and Pendle Locality.
The Home Treatment Team offers 24 hour support to Service Users, and their families/carers, who are suffering from mental health concerns that would otherwise require admission to an acute in patient environment.
The successful applicants will be dynamic and forward thinking, be passionate about working with Service Users experiencing a range of mental health needs and assist in providing a recovery focused community approach to care and treatment. Successful applicants will need to have extended experience of working with Service Users experiencing acute mental health/and or severe and enduring mental illness. They will also need to have excellent communication skills and be able to work independently as well as being committed to supporting a Multi-Disciplinary Team Approach (MDT). The role is providing community care, therefore the successful candidate will need to have be able to travel to allocated visits in the East Lancashire areas.
· To work as part of a team that provides mental health services, to positively promote recovery and social inclusion by focusing on the direct needs of service users, working across boundaries of care, organisations and roles
· To provide support directly and flexibility to service users towards goals defined in his or her care
· To create specific care plans aimed at promoting social engagement, healthy living and independence
· Maintain information and knowledge of and links with community resources and actively support service users to use these resources, thereby countering discrimination and encouraging social inclusion
· Planning and organisation of activities, care plans and programmes of care for Service Users under the Community Mental Health Team/Home Treatment Team in collaboration with the Service User, Carer, Care Co-ordinator/ Crisis Practitioner.
Also to assist with both the assessment & gatekeeping processes
Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.
The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.
