As demand rises with increasing health needs and advances in clinical care, we are responding to the challenges whilst continuing to deliver the highest quality of care. We are developing a new service called 'Home First', which will provide multidisciplinary support for up to 7 days, post-hospital discharge, also working alongside established services to reduce hospital readmissions and to facilitate earlier discharge from hospital through advanced holistic assessment and ensuring patients have the safest best care that is delivered in their home environment.

Croydon Health Services are in a new Alliance Partnership with other health and social care providers in Croydon and the LIFE service is one of our new integrated service models. This means bringing together health and social care teams who work to support people to remain independent and healthy in particular to prevent admissions to hospital and shorten hospital stays.

LIFE is a person centred service available 7 days a week. The Band 4 senior Therapy Assistant will work with a team of nurses, therapists, support workers and social services staff to deliver short-term interventions, rehabilitation and reablement in the community for up to 7 days, following hospital discharge. Our aim is to prevent unnecessary readmission to hospital and facilitate discharge from hospital and most importantly to help patients achieve their maximum potential. The Senior therapy assistant will participate in covering the service across 7 days per week, including bank holidays.

The candidate will also need awareness and knowledge of community services and will need to work closely with the multi-disciplinary team in helping to facilitate safe discharges and promote independence in the persons own home.

The Trust was formed on 1st August 2010 through the integration of Croydon Community Health Services and Mayday Healthcare NHS Trust. Clinical

1. To assess, plan and implement therapy to a designated caseload and adapt intervention to meet client needs. To progress and adapt designated therapy programmes in line with own scope of practice and competency set by the therapy team leaders (which will have been agreed at appraisal) and feedback to the responsible Therapist,taking into account changes in the patients’ health and social needs.This will include:

· Personal activities of daily living i.e. washing, dressing, grooming, toileting

· Domestic activities of daily living i.e. meal and drink

Preparation

· Following a programme of therapy set for individual patients on the caseload

· Mobility and transfer practice

· Assisting the patient with an exercise programme to improve their functional ability

2. To be responsible and proactive when organising caseload in discussion with the other team members.To ensure patients and carers are an integral part of their treatment planning and goal setting.

3. To be responsible for assessment, prescription, issuing and fitting of equipment, whilst demonstrating an awareness of local cost implications.

4. The post holder may be required to assess the level and urgency of therapy need, complete risk assessments and goals with families, individuals, and relevant professionals as required, to determine the level of assistance to be provided directly by the LIFE Service.

5. To use therapeutic handling skills which require dexterity and co-ordination, in the management of adults with acute illness and/or disability with varying mobility/balance/sensory impairment.

6. To work with patients individually or in a group in a variety of settings without direct supervision or presence of a qualified Therapist, including educating patients and carers.

7. To attend Multidisciplinary team meetings as part of the ‘Home first’ service and/or the LIFE therapy community teams and pro-actively offer support/advice and/or accept referrals for patients.

8. The post holder will be required to provide interventions for non-complex patients within the ‘Home first’ service and community LIFE therapy. They will also support the therapist / senior nurse with other more complex patients within these parts of the service, providing verbal and/or written feedback to wider members of the team to support with management and planning of care, reablement and recovery and promotion of independence.

9. To liaise closely with the other members of the team regarding the patients’ progress, informing a senior therapist promptly of any change, deterioration or concerns in the patient’s health/needs.

10. To communicate information to patients and/or carers, from a multicultural, ethnically diverse population where there may be medical barriers to their understanding. To understand the Physical and Psychological barriers to communication related to the patient’s condition and adapt communication according to the patients needs. (This may require the use of interpreters and/or non-verbal communication aids.) To undertake this with groups or individuals.

11. To liaise with and work alongside other professionals / organisations, ensuring effective communication at all times in order to provide excellent patient care.

12. To comply with all Trust, Service and Professional standards, policies and procedures.

13. To maintain a high personal standard of work and to seek advice from senior staff when appropriate.

14. To develop rapport with patients, encourage their interest and participation, in a manner, which promotes dignity, self-esteem and choice. This requires empathy, sensitivity and good interpersonal skills.

15. The post holder will keep accurate and up to date records of the person being supported in line with departmental standards and standards for record keeping.

16. The post holder will be required to use the Trust IT systems to enter the person’s data, activity, service evaluation and any other relevant data collection.

17. The post holder will be required to participate in 1:1 and supervision arrangements organised by the LIFE Manager.

18. The post holder will be required to assess the level of need, complete risk assessments and support plans/ goals with families, individuals, and relevant professionals as required, to determine urgency of therapy need and the level of assistance to be provided directly by the LIFE Service.

19. The post holder will ensure that care plans are written, reviewed and accurate records maintained in the person’s notes and office copies as required.

20. The post holder will escalate any issues or concerns in accordance with Safeguarding procedures and policy.

21. The post holder will work in a flexible mannerin order to respond appropriately to the requirements of the organisation and the needs of the service, service users and patientsover a 7-day period, working a shift-based rota within operating hours.

22. The post holder will require a clean driving licence and be able to travel independently across the Croydon borough.

Nonclinical

1. To undertake administrative duties including answering telephones, taking messages, processing referrals as appropriate including all discharge related paperwork and maintain departmental paper work as required to assist with the smooth running of the team.

2. To use the Trust IT system to enter patient data, activity, service evaluation and any other relevant data collection.

3. To use basic typing and word processing skills.

4. To maintain accurate and up to date statistics as required by the Operational Manager and the Trust.

5. To assist with maintaining stock levels of departmental equipment/stationary as required..

Training and Supervisory

1. To actively participate in departmental training activities.

2. To complete a competency training booklet specific to the role in LIFE

3. To participate in joint patient sessions and specific training to develop a high standard of skills and knowledge, to be able to deal with the complexities of managing patients in this area.

4. To provide advice and to demonstrate own activities to less experienced staff within the workplace.

5. To attend mandatory training in accordance with Trust policy and to attend in-service training and external courses as identified via the Appraisal process.

6. To participate in the Trust’s staff development and appraisal system, and to continue to maintain personal professional development

7. To participate in the induction of new staff as appropriate

8. To attend and contribute to team meetings

Service development

1. To participate in evaluation of clinical practice and/or research projects and the implementation of changes in line with evidence based practice, Therapy guidelines and clinical governance, as delegated by Senior therapist/ Operational Lead.

2. To identify opportunities to improve the service in order to provide the

best possible patient care and to participate in initiatives to improve the service.

3 To actively participate in LIFE team audits and Clinical Governance
