responsible for a defined caseload and will formulate appropriate specific, specialist care and treatment plans; providing skilled and
specialised interventions and care to service users with complex mental health problems.
As a registered Social Worker, OT or Nurse . the post holder will be responsible for working in a way that promotes levels of excellence in the team and operates to the requirements of the service operational policy and strategy, and that safe, evidence based, effective, culturally
competent care is delivered in a timely manner to service users.
The service encourages applications from black and ethnic minority staff. We welcome applications from Social Workers who need to undertake
their ASYE year.
The successful applicant will be responsible for a caseload of service users. They will undertake assessment and review of new referrals. They
will also engage with service users in delivering short term interventions and offer longer term, goal centred interventions through formulating
specialist care and treatment plans.
They will develop professional relationships with other key stakeholders in the service users’ network (i.e Primary Care, Core Teams, Inpatient
and Crisis services, Housing, Carers, Employment, Education etc) and interface with these to support the service user, as well as planning for
step down.
The post holder will also have a sound working knowledge and experience of the Care Act (2014), Mental Capacity Act (2005) and Mental Health Act (2007). They will be required to undertake Care Act assessments for those requiring care packages and presenting cases to panel for funding as required. The CDAT service is a well-established, diverse and experienced Multi-disciplinary team (MDT) comprising of Nurses, Social Workers, OTs,
Psychologists, Psychiatrists, Administrators, Managers, trainees and students.
The service has four pathways (Community MDT, Medical, Psychology and Social Care), which may overlap and offer intensive treatment,
intervention and support for individuals with complex mood disorders such as depression, anxiety and PTSD. These four pathways work alongside side each other with a great deal of peer support and shared learning.
This post is part of the Community MDT pathway and would suit a mental health nurse, OT or social worker who is looking for a challenging yet rewarding role.
You will be offered an induction period as well as regular supervision and appraisal. ‘In-house’ informal training and learning sessions are available on a regular basis, specifically related to support individuals with their work in the service, which are largely facilitated by team members and provides an opportunity to share specific skills and knowledge with the wider team.
The team has been described as warm and welcoming with a ‘friendly atmosphere and an ’open door’ culture where team members are
encouraged to discuss and reflect with managers or senior members of the team.
