Summary
To support the Team leader in responsibility for the day- to day smooth running of the St Catherines at Home service Attend and contribute to the daily handover. Support the Team leader in the Off-duty and shift allocation of the Nursing Assistants Booking and co-ordination of bank staff Work with the Team Leader to process and input referrals on SystemOne. Support the Team Leader in the collection and analysis of data. Support the team with delivering care in the community. Support new staff in using System One Assess, plan, implement, evaluate the delivery of patient centred care, using a holistic approach. Liaise with district nurse with any concerns relating to significant changes in or concerns about patients conditions. Assess patients care needs act accordingly. Maintain accurate patient records in line with the Data Protection Act and NMC guidance, using electronic patient record system. Be able to communicate effectively and sensitively with patients and families and other health care professionals.
