Summary


To be responsible for the care of a group of patients during the shift.
To be one of a team of named nurses, caring for a particular group of patients.
Assists in meeting the requirements of the Clinical Governance programme pertaining to the ward.

Work collaboratively with multi disciplinary team.

Assists in coordinating discharge planning in accordance with London Northwest Healthcare NHS Trust policy.

To assist in the assessment of care needs, development, delivery, and implementation of programmes of care for an assigned patient group, using a validated model of nursing.
To implement appropriate practices in line with the policies of London Northwest Healthcare NHS Trust, to provide skilled/research-based nursing care to meet the identified needs of patients and their relatives.
To work to the agreed standards of care within London Northwest Healthcare NHS Trust, and in collaboration with the Senior Sister/Ward Manager, monitor patient outcomes.
To establish effective communications with medical colleagues and all health care professionals to ensure appropriate nursing input to patient care discussions to ensure safe practice.
To be aware of the policies and philosophies of London Northwest Healthcare NHS Trust, including Health and Safety and ensure all actions are carried out within these guidelines.
1.6 To assist the Senior Sister and Senior Nurse/Ward Manager in maintaining a safe environment for staff, patients, and relatives.
To assist the Education, Learning and Development department in the implementation of nursing research/changes in clinical practice.
Ensures effective participation in Clinical Governance initiatives and actively contributes to compliance with benchmarked standards for quality of clinical care.
To assist in clinical audit in line with Hospital and Divisional requirements.
