Summary

PROFESSIONAL Perform comprehensive assessments of an identified caseload of patients. Plan, implement and evaluate care delivery. Adhere to policies, procedures and clinical guidelines, according to changing health care needs which are agreed for the job role/place of Collect, collate and report information. Maintain accurate patient records by recording and storing all relevant clinical information ensuring it is securely held, accurate, and legible and can be retrieved promptly when required. Maintain accurate patient records and ensure all relevant information is documented in the patients medical and nursing records on EMIS system. Prepare the clinical environment for safe practice by ensuring that the appropriate level of cleanliness is maintained, relevant materials, and any equipment needed for carrying out clinical procedures is available and ready for use. Cleaning of, clearing away and restoring equipment/environment after use. Contribute to the effective and efficient use of physical and financial resources by ensuring that all equipment, stock and supplies are available in the quantity required, and by identifying problems with resources within the team, including offering suggestions and solutions. Promote effective communication and relationships, maintaining confidentiality of information by: Involving parents in the planning and delivery of care. Supporting relatives, professionals, sensitively communicating all information and referring queries to the Specialist Nurses when required. Support and implement product review assessments. Ensure patients and their families are given appropriate verbal and written information regarding plans of care. To undertake home visits as indicated by patients needs and /or when contact in clinics is limited or inappropriate in order to build relationships and provide continuity of their care plan. Promote, monitor and maintain a healthy, safe and secure working environment, ensuring compliance with legal and regulatory requirements. Attend Occupational Health for Health surveillance/vaccination following accidents and for monitoring as laid down by Trust policy. Co-operate with and attend training provided for Health & Safety purposes. Recognising and responding appropriately to urgent and emergency situations. Identify personal learning needs and take steps to pursue personal development plan. Ensure the promotion of safety, wellbeing and interests of patients, staff and visitors to the clinical areas. Enable patient choice and involvement and initiate appropriate action. Provide clinical advice and support to healthcare professionals within and across organisational boundaries. Support and participate in nurse/practitioner led clinics, and Multidisciplinary team meetings. To assist and support Specialist Nurses with school based clinics for Children with Bladder & Bowel Dysfunction and liaise with Special School nurses regarding complex patients.
