Summary


As part of Havens Hospices' multidisciplinary team to provide counselling and emotional support to the children, young people and families using Havens Hospices Care Services to improve mental health and wellbeing following often traumatic life events.

To provide support and education to other teams, agencies and professionals working with children, young people and adults with life-limiting illnesses and following bereavement.
• To work closely with Havens Hospices Care Teams to identify children, young people and families that require psychological support, ensuring good communication and effective joint planning and working.
• To work with patients and families independently within a professional framework and following Havens Hospices policy.
• To ensure that patients and families are aware of the therapeutic supportive services available to them

At Havens Hospices, we care for children, young people and adults with complex or incurable conditions. Our specialist Care Teams can support them and their family living throughout illness, death and bereavement in the comfort of their own home and through our hospice services, Fair Havens and Little Havens.

• To participate in multidisciplinary meetings as necessary.
• To develop activities in accordance with the needs of the service.
• To contribute to service reporting and audits, as necessary.
• To contribute to service outcomes by monitoring and evaluating using a variety of tools.
• To keep records which facilitate communication within the charity following Havens Hospices policy and being mindful of good work practice.
• To adhere to the Service Standard Operating Procedure and highlight any operational variances to the Service Lead.
• To use professional judgement when determining the best course of action including managing safeguarding concerns in a safe and sensitive manner.
