Summary


This is an exciting opportunity for a Patient Flow Assistant to join a small and experienced team working across Division 1 (Surgical Division). You will be able to work on your own initiative and seek help as required. You will join a team of two experienced surgical flow co-ordinators and training/support will be provided.

Providing an efficient administrative service to discharge and transfer co-ordination of Patients

1. To lead on co-ordinating the discharge or transfer arrangements of medically and therapeutically stable patients identified as medically fit for discharge within a designated area. The post-holder will participate in the assessment of after-care needs of clients and relatives/carers, contacting where necessary other members of the multidisciplinary team. The post-holder will independently co-ordinate the safe discharge and transfer of patients back into the community or into ‘step down’ facilities.

2. To collate and provide up to date capacity/bed information within the Trust to support the Capacity Manager, the Trust Management Team and on-call managers with regard to all specialities, aiding decision making of the Capacity Management Team and informing Divisions and Directors of the Trust Capacity Status.

The post holder is responsible for liaising with patients, relatives, medical, nursing and Allied health professional hospital staff both within New Cross and other hospitals, community and intermediate care staff, adult social care services across several local authorities and the public on all levels.
