Summary

Main Duties and Responsibilities:

• Acting as Allocated Support Worker to support individual young people, centring the voice of the young person so that young people take a lead role in determining their support, and taking a trauma informed approach to working with young people.
• Developing strong relationships with each young person through understanding them as individuals with their own life experiences, cultures, challenges and aspirations. This will include conducting a minimum of one – one hour – support session every week with allocated young people (in addition to appointments, welcome, advocacy, move ins etc). Time permitting as much time as possible will be spent with YPs providing support and deepening relationships.
• Welcome and induct new young people following the Young People’s Welcome Guide and inducting them to the physical and social environment of the placement (e.g. appliances, physical safety and security, meeting and living with other young people)
• Assess young people’s individual needs by working with the young people and utilising referral information to develop an individual support plan.
• Ensuring that the support provided, and individual support plan, is reviewed and updated, working with the individual young person and in consultation with relevant third parties (including the local authority). Informing the Outreach Team Leader if there are concerns, including if the young person needs more intensive support.
• Monitoring the impact that support provided is having on young people’s outcomes and reviewing and updating the individual support plan as needed to ensure that support is effective
• Working with each young person to complete the Independent Living Skills Programme covering cooking, cleaning, personal hygiene, shopping, using local transport etc.. This is to ensure that our young people transition into supported living (and where applicable life in the UK) and transition out of supported accommodation successfully: feeling supported and positive about their future opportunities because of the support received.
• Actively supporting young people with welfare checks, how to keep safe, support with physical health (opticians, dentists, GPs), physical activity (e.g. leisure centres, gym, football) mental health (including trauma informed practice), educational, training and employment and legal needs.
• Making representations on behalf of the young person if the support worker believes the local authority or Bright Sky as a company is failing to provide the support set out in the child’s relevant plans with a view to ensuing the young person’s needs are met. Where necessary this can be done with support from the Outreach Team Leader with our partners at the Local Authority.
• Ensure that young people are supported to understand the impact of any experience of neglect and abuse.
• Supporting young people to have appropriate contact with their families.
• Working in close partnership, including attending meetings and sharing information, with the local authority to best serve the needs of each young person and in doing so, securing the excellent reputation of the company. This requires demonstrating outstanding and prompt communication and following up on all actions promptly.
• Ensure that young people are keeping the premises clean, are respecting their accommodation and maintaining the safety of themselves and others – though applying the behaviour management policy and other approaches where necessary.
• Completing weekly reports on time and to a high standard, containing relevant information.
• Ensuring young people’s files are kept up to date with all progress and correspondence.
• Identify and act upon signs that a child is at risk of abuse, neglect, exploitation or any other harm. Including where relevant escalating concerns, or risks, to the line manager promptly.
• Support young people to identify, access and receive their entitlements in a way that helps them to receive their entitlements in the future.
• Actively encouraging young people to engage with relevant third party agencies e.g. community groups, clubs, third sector organisations offering support for care leavers and UASCs (including advocacy and training)
• Organising trips and activities for individual young people and / or groups every 6 weeks, managing your budget allocation for these activities well for best value for money and support
• Implement all Brightsky processes and policies, including but not limited to – Safeguarding, Behaviour Management, Missing Child, Complaints
• Actively engage and progress with training and personal development required for the role, including achieving NVQ Level 4 or equivalent in Health and Social Care.
• Modelling positive behaviours and a positive attitude e.g. time keeping, office and car cleanliness, professionalism, team work
• Additional tasks as and when required.
