Integration and Transformation Manager – Band 8a

We are a relatively new directorate within a family of the NHS LLR Integrated Care Board. We cover interconnected portfolios such as primary care, home first, long term conditions, elective care and cancer, urgent and emergency care, children and young people & mental health, learning disability and autism.

The I&T Manager will:

I. Directly manage multi-agency programmes and projects relating to the delivery of integrated care at system, place and neighbourhood level.

II. Support the development of primary care and primary care networks ensuring that the sector becomes resilient and can deliver integrated services within a wider network of providers.

III. Support and contribute to system wide restoration and recovery planning
