The post holder will work under the direction of the Transplant nurse in Mechanical Circulatory Support and will include; Supporting the Nurse Specialist in the management of patients with Mechanical Circulatory Support (MCS). This will include assisting the Nurse Specialist with the management of the patient in an inpatient and outpatient setting. Learning how to assist the Nurse Specialist in complex situations with patients with MCS. Liaison with nursing staff and MDT to deliver individualised, compassionate care to patients, care givers and companions. Work with the Nurse Specialists to facilitate high quality, individualised care to patients/clients and relatives in collaboration with the multi-disciplinary team. With supervision from a Nurse Specialist, assist with clinical procedures and investigations. Assist the Nurse Specialist in the preparation of tests and result for outpatient clinic. Safely and competently escort stable and designated MCS patients to and from departments within the hospital and local vicinity. Escort stable and designated MCS patients to other hospitals for procedures. To competently complete and document basic driveline dressings and report any changes to a Nurse Specialist as necessary. To learn to competently set up theatre trolleys for MCS implants. Ensure that equipment is cleaned, re-stocked or disposed of as per policy. Ensure phone and e-mail enquiries to the MCS office are dealt with appropriately and in a timely fashion. To assist the Nurse Specialist with the collection of information from local hospitals including blood results, procedure results and clinic letters. To assist with safe and well communicated patient discharge into the community. To assist in the preparation of paperwork for national VAD database and other research projects. To participate in collecting the necessary data for these projects. The post holder is expected to carry out all relevant forms of care with support of the specialist nursing team and clinical specialists.
