Summary


The Royal Wolverhampton Planned Care Teams are highly motivated and passionate about providing high quality, evidence based care to patients’ in their own homes. Possessing a ‘can do’ attitude and a commitment to improving the way healthcare is delivered. We need exceptional individuals with a passion for delivering high quality care with a focus on patient safety.

You must be a Registered Nurse. The District Nursing Specialist Practitioner Qualification or relevant degree e.g. palliative care, wound care, chronic disease is desirable. You will be IT literate and be confident utilizing applications to enable remote consultation with patients/to attend MDTs.

The staff work closely with members of the wider multi-disciplinary teams including local Hospices, Urgent care team, Care Coordination Hub, Ambulatory Teams and Support Care Teams.

To be professionally accountable and maintain the standards of professional practice as set by the appropriate regulatory body applicable to your profession or role.

To deliver compassionate person-centred care, and treat patients, service users, their families and their carers with consideration, dignity and the respect you would want for yourself or your family.

To support and deputise for the Senior Sister / Charge Nurse in ensuring the standards of professional practice as set out by appropriate regulatory body applicable to your profession or role are maintained.

Utilise clinical proficiency in delivering a high standard of nursing care. Provide managerial and leadership skills to support the Senior Sister / Charge Nurse and nursing team to deliver continually improved standards and efficient and effective use of resources.
