Description
We are currently seeking a dedicated and compassionate Social Worker to join our team at Cheam First Response. As a Social Worker, you will play a vital role in supporting and empowering adults in the local community. You will be responsible for assessing their needs, developing care plans, and providing ongoing support and guidance. Through your work, you will make a significant impact on the lives of individuals and their families.
Responsibilities
Conduct assessments to determine the needs and eligibility of adults in the local community
Develop personalized care plans based on the assessed needs and in consultation with the individuals and their families
Provide ongoing support and guidance to adults, ensuring their welfare and promoting their independence
Liaise and collaborate with other professionals, such as healthcare providers and community organizations, to coordinate comprehensive care
Advocate on behalf of adults, ensuring their rights and needs are met within the context of legal and ethical frameworks
Monitor and review the progress of adults, making adjustments to care plans as needed
Provide crisis intervention and support to individuals and families in urgent situations
Requirements
Bachelor's degree in Social Work or a related field
Valid license or certification to practice as a Social Worker
Proven experience working with adults in a social work setting
Knowledge of relevant legislation, policies, and procedures related to adult services
Strong assessment and case management skills
Excellent communication and interpersonal skills
Ability to work collaboratively in a multidisciplinary team
Strong organizational and time management abilities
Compassionate, empathetic, and non-judgmental mindset
Proficiency in computer systems and software used in social work practice
Hours Per Week: 35.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £22.60
