This is an exciting time in Community Services and in particular Integrated Care.
We are looking for a Community Matron who is creative, highly engaged, and flexible to join our friendly Integrated Care Team within Provide. Mid Essex is located 30 miles northeast of London, Our community covers 130 square miles and includes the only city in Essex, plus open countryside and attractive villages. Chelmsford City, offers first class shopping, wining & dining and visitor attractions, leisure opportunities and recreational open spaces.
You will work as part of a multidisciplinary team supporting them in providing high quality care to patients.
You may be required to support services within your locality across the Mid Essex region at other Organisational sites, should the need arise, therefore access to a car is essential. Our operational hours are 08.00 - 23.00, seven days a week, the post holder will be required to Monday-Friday 0800 - 1600.
• The post holder will act as a professional and clinical role model for end of life care, dementia and the out of hours nursing service, demonstrating strong leadership and upholding the values of the organisation and profession as well as behaviours to facilitate a culture of trust, openness, integrity, kindness and mutual support.
• They willact as an ambassador for Provide and demonstrate excellent customer care.
• This role involves working collaboratively with hospices, community palliative care teams, primary care, secondary care, care homes, dementia intensive support service and voluntary organisations. Supporting local and national palliative care strategic planning to provide seamless integrated care for palliative patients and those important to them.
• The post holder will work with community teams to support and advise people living with dementia and those important to them at each stage of disease progression or as patient need changes and until end of life.

• Participate in on-going face to face palliative care and dementia service.
• Participate in on-going service development through audit, research and education.
• Participate in the wider end of life care and dementia strategic education programme within Mid and South Essex Community Collaborative.
• Support teams to actively engage people with dementia and cognitive impairment and their carers in all stages on intervention, ensuring that their views are acknowledged and considered.
• Organise and chair regular meetings with community services and appropriate stakeholders to educate staff enabling best evidence based practice to be delivered.
