Summary

To manage a defined caseload of complex and challenging clients who present as high risk and will have demonstrated this through the nature and gravity of their offending. You will be expected to manage the conflict between the therapeutic alliance whilst keeping within the statutory and legal framework. You will be accountable for the assessment of care needs, risk assessment, development, implementation and evaluation of treatment packages and ensure that reviews take place within the Trust CPA policy. Provide care coordination and joint working for a designated caseload of community clients who have committed serious offences and may be subject to formal statutory supervision e.g. conditionally discharged by the Home Office and subject to Community treatment orders. Develop and implement evidence-based interventions. Act in the capacity as social supervisor for conditionally discharged patients Ensure clinical effectiveness in working with patients, families, and carers, including people who disengage from services, by facilitating engagement and therapeutic co-operation using flexible and responsive engagement strategies. Preparation of social circumstances reports for Mental Health Review Tribunal reports as requested. Encounter distressing accounts of offending behaviour whilst undertaking assessments as well as in the course of your day-to-day duties e.g. extreme violence or of serious sexual assault Provide a range of social care interventions including the preparation of reports for funding panels, mainstream housing, benefits etc Assessing the suitability of residential placements. Coordinate MHA assessments in the community. Provide monitoring of out of area placements ensuring attendance at CPAs for service users that have been admitted To provide specialist input and advice to forensic mental hostels within the Bracton centre's catchment area. Works within codes of practice and professional guidelines and is accountable for own professional practice. Required to act within broad occupational policies without guidance. To participate in reflective practice within the supervision framework. Administers prescribed medication safely as appropriate and in accordance with Oxleas policy and NMC guidelines. Occasional/frequent exposure to distressing, volatile and unpredictable situations e.g. verbal and challenging behaviour The full-time position will focus on the effective facilitation of the depot and Clozapine clinics incorporating our Physical Health Pathway to provide a seamless service to patients:1. To deliver physical health assessments and interventions to patients under the service inclusive of phlebotomy.2. To assist in the management of a clozapine clinic.3. To administer long-acting injections and facilitate clozapine monitoring.
