Summary
The following are the core responsibilities of the Lead Practice Nurse: To act as the Infection Prevention Control (IPC) lead and to work to the IPC Policy providing audits, training, support and evidence of compliance to the practice manager To lead on and deliver chronic disease management for patients To lead the team in providing routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF To lead on the practice's vaccination and immunisation programmes for adults and children To lead on cytology To lead on wound care management and dressings To prescribe medication for patients in accordance with PGDs and own scope of practice To review patient medications to enhance compliance To provide guidance to patients on the use of prescribed and over-the-counter medications To develop, implement and embed health promotion and wellbeing programmes To develop, implement, embed and participate in well woman and man clinics To support the team in delivering anti-coagulation clinics To support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice) To support the clinical team with all safeguarding matters, in accordance with local and national policies To manage and support the nursing team to deliver all aspects of care and governance Responsible for implementation & update of clinical policies and procedures for the team to follow Responsible for ensuring relevant PGD's/PSD's continue to be signed & in date Ensuring smooth running of all clinics Appraisals, performance managing, continuing support of team members Implementing new clinical initiatives & ways of working, in order to meet the needs of the service Daily administration duties as is necessary to fulfil the role
