Summary


We are seeking an enthusiastic Registered Nurse to join our Community Nursing Service in North East Essex Community. In this role, you will play a vital part in delivering high-quality healthcare to patients within their own homes as part of a multidisciplinary team. The aim is to promote self-management and independence in daily activities, ensuring patients receive the support they need.

As a Registered Triage Nurse within our Community Nursing Service, your main aim is to provide telephone triage to all referrals and contact to the community nursing service. You will play a critical role in assessing patients’ health conditions and determining the urgency of their care.

Interviewing Patients: Conduct thorough interviews with patients to gather information about their medical history and current symptoms.

· Triage Priority Assignment: Based on the interview and examination findings, assign a triage priority to patients, ensuring the most urgent cases are addressed promptly.

· Communication: Coordinate with physicians, nurses, and other care team members regarding patients’ statuses, ensuring seamless transitions of care.

· Health Information Updates: Update patient health information systems with relevant notes, ensuring accurate and up-to-date records.

· Staying Current: Keep up-to-date with advancements in community nursing and best practices to ensure high-quality care delivery.

· Adherence to Standards: Follow infection control, sanitation, patient safety, and confidentiality standards to maintain a safe and secure environment for patients and staff.
