Our Integrated and specialist teams of Nursing and Allied Health professionals deliver care to the people who present with significant physical health care needs in various settings including clinic based, community inpatient, hospices to care in patient's own home, including care homes, enabling patients to be independent, self caring and live their optimum lives, reducing hospital admissions.

Services range from Community Nursing, including specialist practitioners to public health and health promotion such as school nursing and health visiting, with many specialist community services designed to support care pathways such as Physiotherapy, OT, speech and Language, Dietetics and Podiatry.

Supporting efficient running of these services are administrative and managerial roles. All roles, are vital in transforming community health services, ensuring high quality care is provided both now and into the future based around the needs of our patients - together we all make a difference.

An exciting opportunity has arisen for part time (4 days per week) Specialist Heart Failure Nurse to join our excellent Community Cardio Respiratory Team in Mid Notts, helping to support caseload management of patients with a diagnosis of Heart Failure.

This role is based at Ollerton health Circle and cover the Mid Notts District. Providing Care and treatment for patients who have been diagnosed with left sided Heart Failure.

You will be expected to support caseload management of patients with Heart Failure, proving care plans as appropriate, working closely with members of the Cardio Respiratory Team and the wider MDT- including our acute colleagues, GPs and other health professionals in order to maximise patients quality of life and encouraging self management. This care takes place within community clinic settings and patients own homes /usual place of residence.

We are looking for a passionate, dynamic and flexible individual who are able to adapt to the changing demands and workload.

The service is a 5 day service operating from 8.00-5.00pm, due to the nature of this role, it is essential that you are able to travel independently across the county and have access to a suitable vehicle for business purposes.

This is a Band 6 development post with the potential for the role to be a Band 7 once competencies and NMP V300 have been attained.
